WHY DOES SNORING HAPPEN?
Snoring occurs when your upper airways narrow too much, causing turbulent airflow. This then makes the surrounding tissues vibrate, producing noise.
SNORING CAUSES:
Many things can cause snoring. Causes can include:
* being overweight
* alcohol
* medications - like sleeping pills
* very low soft palate
* nasal and sinus problems
* smoking
* enlarged adenoids, tonsils
* hormonal changes - pregnancy, menopause
IS SNORING SERIOUS?
Snoring may be a sign of sleep apnea, which can cause you to stop breathing during your sleep. If you think that is happening to you, please see your doctor for further tests.

WHAT IS TINNITUS?
Tinnitus is the name for when you hear noise in your ears or head, but it doesn't come from an outside sound. It is a very common problem. People with tinnitus usually hear ringing, buzzing, or whistling sounds.
WHY DOES TINNITUS HAPPEN?
Tinnitus is not a disease, but a symptom of a problem with your hearing system. The problem can be anywhere from your ears to your brain. Different people experience tinnitus differently.
TINNITUS CAUSES:
Many things can cause tinnitus, and we don't fully understand them all yet. Causes can include:
* ageing
* loud music or noise
* medications
* ear wax
* infections
* injury
* illnesses such as diabetes or Ménière's disease.
IS TINNITUS SERIOUS?
Tinnitus is not usually serious, but if it's a new symptom, it is sensible to get your GP to check it. This is especially important if your tinnitus is in just one ear, you have sudden hearing loss or you notice it is like your heartbeat/pulse.
INVESTIGATIONS:
Your doctor will look into your ears and will usually suggest a hearing test. Occasionally you may need further tests, such as blood tests or a scan, to find the cause.
TINNITUS TREATMENT:
If your doctor finds a cause such as ear wax or Ménière's disease, it will need treating.
For most people there isn’t an underlying cause to treat.
Most people can learn to cope with their tinnitus using simple self help measures.
If the tinnitus is an ongoing problem options include:
* hearing aids
* sound devices to mask the sound
* counselling for stress.
* An audiologist can help advise on what might suit you.
Self care for tinnitus:
* Reduce your exposure to loud noises.
* Quit smoking.
* In a quiet setting, a fan, soft music or low-volume radio static may help cover up the noise from tinnitus.
* Manage stress, since stress can make tinnitus worse.
* Dietary management of hyperinsulinaemia
OTOTOXIC MEDICATIONS:
Ototoxic means a drug or chemical that can damage the inner ear. Ototoxic medicines include -
* Certain Antibiotics, eg amikacin, gentamycin, kanamycin, neomycin, netilmicin, streptomycin, tobramycin, azithromycin, clarithromycin, erythromycin, vancomycin
* Antimalarials eg chloroquine, mefloquine, quinine
* Chemotherapy medications eg carboplatin, cisplatin, cyclophosphamide
* Diuretics (water tablets) eg bumetanide, ethacrynic acid, furosemide, torasemide
* Non-steroidal analgesics eg acetaminophen (paracetamol), aspirin (high doses), ibuprofen, indomethacin, salicylates

What is Carpal Tunnel?
The carpal tunnel is a narrow passage in your wrist, through which your median nerve and the tendons to your thumb and fingers pass. If you have Carpal Tunnel Syndrome, your median nerve is squashed, from pressure on it.
What are the Symptoms of Carpal Tunnel?
* Numbness or tingling in your thumb and fingers
* Pain in your wrist, which can shoot into your fingers, or sometimes up to your elbow.
* Muscle weakness, especially if you have to grip something.
* Night symptoms - eg burning pain, or when doing the same task for a long time, like long-distance driving.
What are the Causes of Carpal Tunnel?
We don't always know, exactly. Anything that causes swelling in the carpal tunnel can cause it. It can happen if your tendons become inflamed, putting pressure on your median nerve. It can happen in one or both wrists. It is more common in women.
Some conditions that can cause your wrists to swell can also cause carpal tunnel syndrome, or make it worse. They include:
* pregnancy
* obesity
* fluid retention
* diabetes and hypothyroidism
* rheumatoid arthritis and connective tissue diseases
* wrist wounds or broken bones.
* after forceful or repetitive movements of your wrist or hand.

What is Gout?
Gout is a common form of arthritis that causes your joints to swell painfully. It's caused by a chemical in your blood called uric acid. If there's too much uric acid in your blood, it turns into tiny, sharp, glass-like crystals. The crystals collect in your joints and irritate the tissues causing inflammation, swelling, and pain – a gout attack.
Gout can damage your joints and your kidneys if it's not treated.
Who gets Gout?
It affects around 1% of the population. It tends to run in families.
Generally, uric acid levels are elevated for 20 years before gout develops. In men uric acid levels rise at puberty so gout is often diagnosed in their 4th to 6th decade of life. Oestrogen protects young women from gout; uric acid levels rise at menopause and gout is often diagnosed in their 6th to 8th decade of life.
High Purine Foods:
* Alcoholic beverages (all types)
* Some fish, seafood and shellfish, including anchovies, sardines, herring, mussels, codfish, scallops, trout and haddock
* Some meats, such as bacon, turkey, veal, venison and organ meats like liver and kidney
* Yeast extracts such as Marmite and Vegemite
* Meat extracts such as Bovril.
High Uric Acid Causes?
* A diet that is high in animal-based protein, especially meat and seafood.
* Moderate to high alcohol intake, especially of beer.
* High intake of fructose, a sugar found in sugar-sweetened soft drinks and fruit juices (apples and oranges).
* Being overweight
* Kidney disease makes it harder to get rid of uric acid
* Some types of anaemia (Haemolytic anaemia and Pernicious anaemia).
* Medications
Medications that cause high Uric Acid levels:
• Furosemide eg Lasix
• Thiazide diuretics eg Bendrofluazide
• Immunosuppressants eg Ciclosporin
• Low dose Aspirin

WHAT are Kidney Stones?
Kidney (renal) stones are a common and painful medical problem. They happen when there are too many crystals in your urine, which stick together and form hard stones.
These stones can be as small as a grain of rice or as large as an apple. They can happen anywhere in your urinary system, such as your kidneys, one of the tubes that drain urine from your kidneys (called your ureters) or your bladder.
What CAUSES Kidney Stones?
There are several reasons you might get kidney stones.
1. A lack of chemicals in your urine can allow the crystals to stick together to form stones.
Kidney stones happen more often when your urine becomes concentrated. This means that people who live in warm climates have a higher risk of developing kidney stones as they may become dehydrated.
2. Eating certain foods such as foods high in salt increases the risk.
3. Having a family or personal history of kidney stones is also an increased risk.
4. Some kidney stones happen because you have high levels of chemicals such as calcium, uric acid or oxalate in your blood.
These lead to different types of kidney stones such as:
CALCIUM STONES
Calcium stones are the commonest type. They happen if you have a lot of calcium in your blood. You're more likely to get calcium stones if you don't drink enough fluid and you eat a lot of salt.
URIC ACID STONES
These stones are more common in men than in women. They happen because your urine is too acidic and there's a lot of uric acid in your blood. You're more likely to get uric acid stones if you don't drink enough fluid and you eat a lot of fish and red meat, especially offal meats such as liver, kidney and brains.
STRUVITE STONES
These are slightly more common in women who get repeated urine infections. They're made up of magnesium, ammonium and phosphate.
What are the SYMPTOMS of Kidney Stones?
Some people have no symptoms or mild symptoms such as aching in their lower back and urine infections. Sometimes the stones are found by chance when having an X-ray or scan for another reason.
If the stone moves from your kidney towards your bladder, you can get severe pain. You can also feel like you need to pass urine more often.
Sometimes people can pass kidney stones in their urine and have no or few symptoms. But kidney stones can get stuck and cause a blockage. This is called renal colic, and can be very painful.
The symptoms of renal colic include:
* pain that comes and goes.
* pain in your lower back or the side of your abdomen
* blood in your urine (your urine can look pink)
* feeling sick and vomiting
* a high temperature (fever), which can happen if you get a urine infection.
If you have any of these symptoms, see your GP or after-hours healthcare provider as soon as possible for tests and pain relief. If you're very unwell, you may need to go to hospital.

* blood test
* urine test
* CT Scan
How to PREVENT Kidney Stones:
Most kidney stones are made of calcium and oxalate. They form when your urine has too much calcium and oxalate and not enough fluid.
Oxalate is a naturally occurring substance that comes from many plants. It's also made in the body.
To prevent Calcium Oxalate Stones:
1. Drink plenty of fluids (water)
2. Reduce your salt intake
3. Eat plenty of calcium rich foods
4. Choose small servings of animal protein foods
5. Avoid high-dose vitamin C supplements

Zinc Supplement side effects:
• Headaches.
• An unpleasant taste.
• Tummy ache.
• Feeling sick (nausea), or being sick (vomiting).
• Diarrhoea.
• Indigestion.
• Tiredness.
Never take more than the recommended amount, due to the risks of excess zinc.
Cautions of Zinc Supplementation:
• Zinc may accumulate in Acute Kidney Disease
• Those with Haemochromatosis may absorb larger amounts of zinc.
• Excess zinc supplementation can interfere with iron and copper absorption.
• It can also reduce magnesium and calcium absorption.

What is Sleep Apnea?
Obstructive sleep apnoea (known as OSA) is a common sleep problem where your airways narrow in the night, which stops your breathing for short spells when you are asleep.
What causes Sleep Apnea?
Factors that increase the risk of developing Sleep Apnea (OSA) include:

What does Metoprolol do?
Metoprolol is used to treat some heart problems and high blood pressure. It is also sometimes used for other conditions such as migraines.
How should you take Metoprolol (Lopressor)?
Take metoprolol regularly as directed with a glass of water. The controlled release (CR) or sustained release (SR) tablets may be halved if needed, but do not crush or chew them.
What if you forget a dose?
Take the missed dose as soon as possible. If it is close to the time for your next dose, skip the missed dose and carry on as normal. Do not take two doses at the same time.
Can you take other medicines with Metoprolol?
Some medicines available without a prescription may react with Metoprolol including: anti-inflammatories, such as diclofenac (e.g. Voltaren®), ibuprofen (e.g. Nurofen®), or aspirin (e.g. Disprin®, in doses used for pain relief). These can also be found in some cold and flu medicines (e.g. Nurofen Cold and Flu®). diphenhydramine (e.g. Unisom SleepGels®) Tell your pharmacist or doctor about all medicines or treatments that you may be taking, including vitamins, herbal products or recreational drugs.
Is there anyone who can't take Metoprolol?
Tell your doctor if you have liver or circulation problems, asthma or diabetes.
Tell your doctor if you are pregnant, planning to become pregnant, or breastfeeding.
Do not stop taking Metoprolol without talking to your doctor first.
Metoprolol side effects:
Please tell your doctor immediately if you have:
* Fainting
* Swollen feet or legs, or have shortness of breath
* Low mood
Tell your health professional if troublesome:
* Tiredness or weakness
* Trouble sleeping or strange dreams
* Cold hands and feet, tingling or numbness in your extremeties
* Changes in dexual function
* Stomach upset
If you feel lightheaded or dizzy after standing up, try standing up slowly and tell your doctor if it is severe or continues.
Will I still be able to drive?
Do not drive or use machinery if you experience any side effects that may make it difficult to do so, such as dizziness.
What strengths does Metoprolol come in?
Metoprolol Succinate (long acting, once daily) comes as 23.75mg, 47.5mg, 95mg or 190mg tablets.
Metoprolol Tartrate (short acting, twice daily) comes as 50mg, 100mg, 200mg tablets.

* dread or avoid everyday activities, such as meeting strangers, starting conversations, speaking on the phone, catching public transport, or talking to shop assistants
* avoid or worry a lot about social activities, such as group conversations, eating with company, and going to parties
* worry about doing something in public that you think is embarrassing, such as blushing, sweating, or making some social error
* find it difficult to do things when others might be watching – like eating, drinking, or writing
* often have symptoms such as feeling queasy, hot, sweating, trembling or a pounding heartbeat (palpitations) when anticipating or entering social situations
* have panic attacks panic attacks, where there is an overwhelming sense of fear and anxiety (usually only for a few minutes).
What are the CAUSES of Social Anxiety?
* A specific incident or event – if you experienced shame or humiliation in a particular situation, you may develop anxiety about similar situations or experiences that you associate with that event.
* Family environment – parents who were very worried or anxious when you were growing up can have an effect on the way you cope with anxiety in later life, and you may even develop the same anxiety as a parent or older sibling.
* Genetics – some people appear to be born with a tendency to be more anxious than others, which can develop into an anxiety disorder.
* Long-term stress – this can cause feelings of anxiety and depression, and reduce your perceived ability to cope in particular situations. This can make you feel more fearful or anxious about being in those situations again, and over a long period, may increase your anxiety about those situations.
Sleep Hygiene Tips:
* Go to bed at the same time each night and get up at the same time each morning, including on the weekends
* Make sure your bedroom is quiet, dark, relaxing, and at a comfortable temperature
* Remove electronic devices, such as TVs, computers, and smart phones, from the bedroom
* Avoid large meals, caffeine, and alcohol before bedtime
* Get some exercise. Being physically active during the day can help you fall asleep more easily at night.

IBS Treatment (Medications):
1. Antispasmodics: The best evidence for treatment include Cimetropium (Alginor), Peppermint oil, Pinaverium (Dicetel and Eldicet) and Trimebutine. The pain may ease with medication but may not go away completely.
2. Constipation Medication: A medicine called Linaclotide (Linzess) works differently to other medicines for treating constipation. It has been shown to reduce pain, bloating and constipation symptoms.
3. Anti-diarrheal Medication: Loperamide (Imodium) may be useful if diarrhea is the main symptom.
4. Anti-bloating: Peppermint Oil
5. Anti-depressant Medications: A Tricyclic antidepresaant at low dose, eg Amitriptyline is sometimes used to treat IBS. They help with the pain of IBS. SSRI antidepressant medicines eg Fluoxetine are also used for IBS.

What is Metformin for?
Metformin is used to treat Diabetes, Pre-diabetes and Polycystic Ovarian Syndrome.
How should you take Metformin?
Take metformin regularly as directed with food and a glass of water.
Why is the doctor increasing my Metformin dose?
To get the best control of your blood sugar levels. Some people only need 500mg of Metformin once a day. While others need a maximum of 2x 500mg, three times a day.
What if you forget a dose?
Take the missed dose as soon as possible (with food). If it is close to the time for your next dose, skip the missed dose and carry on as normal. Do not take two doses at the same time.
Can you take other medicines with Metformin?
Tell your pharmacist or doctor about all medicines or treatments that you may be taking, including vitamins, herbal products or recreational drugs. The below medications can interact with Metformin:
• acetazolamide
• diatrizoate
• dichlorphenamide
• ethanol
• gatifloxacin
• iodamide
• iodipamide
• iodixanol
• iohexol
• iopamidol
• iopromide
• iothalamate
• ioversol
• ioxaglate
• ioxilan

Is there anyone who can't take Metformin?
Tell your doctor if you have kidney problems, liver or heart problems.
Tell your doctor if you are pregnant, planning to become pregnant, or breastfeeding.
Metformin side effects:
The more common side effects of metformin include:
• heartburn
• stomach pain
• nausea or vomiting
• bloating
• gas
• diarrhea
• constipation
• weight loss
• headache
• unpleasant metallic taste in mouth
The diarrhea is driving me crazy!
Metformin had the lowest adherence rate because of side effects like diarrhea. For some people switching to the "extended release version of Metformin" helps.
**However, recently (June 1, 2020) - all lots of metformin hydrochloride extended-release tablets 500 mg from numerous drug manufacturers have been recalled, after tests by the U.S. Food and Drug Administration showed higher NDMA levels than allowed.
N-Nitrosodimethylamine (NDMA) is believed to cause cancer in people.**
What strengths does Metformin come in?
It comes as 500, and 850mg tablets.
What are the symptoms of Hypoglycaemia (Low blood sugar)
* An irregular or fast heartbeat
* Fatigue
* Pale skin
* Shakiness
* Anxiety
* Sweating
* Hunger
* Irritability
* Tingling or numbness of the lips, tongue or cheek
As hypoglycemia worsens, signs and symptoms can include:
* Confusion, abnormal behavior or both, such as the inability to complete routine tasks
* Visual disturbances, such as blurred vision
* Seizures
* Loss of consciousness
