Proton M Rocket Launch Failure and Explosion July 2013, a Proton-M/DM-03 carrying three GLONASS satellites failed shortly after liftoff. The booster began pitching left and right along the vertical axis within a few seconds of launch. Attempts by the onboard guidance computer to correct the flight trajectory failed and ended up putting it into an unrecoverable pitchover. The upper stages and payload were stripped off 24 seconds after launch due to the forces experienced followed by the first stage breaking apart and erupting in flames. Impact with the ground occurred 30 seconds after liftoff. The preliminary report of the investigation into the July 2013 failure indicated that three of the first stage angular velocity sensors, responsible for yaw control, were installed in an incorrect orientation. As the error affected the redundant sensors as well as the primary ones, the rocket was left with no yaw control, which resulted in the failure.[21] Telemetry data also indicated that a pad umbilical had detached prematurely, suggesting that the Proton may have launched several tenths of a second early, before the engines reached full thrust.

At its launch attempt on 6 December 1957, at Cape Canaveral Air Force Station, the booster ignited and began to rise, but about two seconds after liftoff, after rising about 1.2 m (four feet), the rocket lost thrust and fell back to the launch pad. As it settled, the fuel tanks ruptured and exploded, destroying the rocket and severely damaging the launch pad. The Vanguard 1A satellite was thrown clear and landed on the ground a short distance away with its transmitters still sending out a beacon signal

Delta II exploded 13 seconds after launch. The explosion occurred when a damaged solid rocket booster casing ruptured and triggered the vehicle's flight termination system. No one was injured, and the launch pad itself was not seriously impacted, though several cars were destroyed and a few buildings were damaged GPS IIR-1 or GPS SVN-42 was the first Block IIR GPS satellite to be launched. It was to have been operated as part of the United States Air Force Global Positioning System. It was launched on 17 January 1997, and was destroyed 13 seconds into its flight due to a malfunction of the Delta II launch vehicle that was carrying it.

Explorer S-1, also known as NASA S-1 or Explorer 7X, was a NASA Earth science satellite equipped with a suite of scientific instruments to study the environment around the Earth. The spacecraft and its Juno II launch vehicle were destroyed five seconds after launch on 16 July 1959, in a spectacular launch failure caused by complications with the launch vehicle's power supply. A relaunch of the mission in October 1959, Explorer 7 (S-1A), was successful.
