Agents are following Bertrand Zobrist (Ben Foster), a geneticist, through the streets. Christoph Bouchard (Omar Sy) confronts Zobrist as he runs to the top of a tower. Bouchard demands Zobrist to tell him where something is important and potentially dangerous. Instead, Zobrist drops out of the tower's window and strikes two roofs before crashing to the ground.

Professor Robert Langdon (Tom Hanks), is currently in hospital. A head injury has left him with disturbing visions of a hellish scene where people are seen writhing in pain while others are burned and others have their heads turned backwards. Langdon hears voices, and sees someone wearing a hood. Langdon wakes up to find Dr. Sienna Brooks (Felicity) who claims she has been a fan since childhood. Langdon is told by Sienna that he has retrograde amnesia due to the wound. Soon after, Vayentha (Ana Ularu), an assassin comes in disguised to be an officer. She shoots one doctor, then attempts to enter Langdon's bedroom to kill him. Langdon and Sienna manage to escape, but Langdon's blood is dripping from his arm after the IV was pulled off. Langdon is able to get into a taxi and they both pass out.

Langdon awakens in Sienna’s home. He asks Sienna for coffee even though he has forgotten the word. Langdon checks his email as she heads to the counter to find it. Langdon finds one from Ignazio Busoni, Gabor Urmai. The message informs Ignazio that he is safe and that Langdon has apparently stolen something. It also contains the message "Paradise Twenty Five".

Langdon and Sienna discover a Faraday tiper among their possessions. It contains the famous "Map of Hell", by Sandro Boticelli. This map was inspired by Dante’s "Inferno". It has letters and Zobrist’s name. They watch a video in which Zobrist talks about his views on humanity and how the world should be cleansed. The two discover that Zobrist had planned to spread a virus to the world to wipe out billions before he committed suicide. Langdon talks to someone over the phone from the U.S. consulate to discuss being picked up from his bedroom. But, he later discovers that Vayentha, and other officers, are there for him. This forces him and Sienna back to running.

Langdon and Sienna attempt to decipher the meaning of "cerca trova", an Italian expression that means "seek and locate". Langdon is also being sought out by associates from the World Health Organization, including Elizabeth Sinskey (Sidse Babett Knudsen). She tells her men not to trust anyone and to find Langdon using any means necessary.

Vayentha calls her associates to talk to "The Provost". This Harry Sims (Irrfan Khan), CEO at The Consortium. Sims is contacted by one of his men about the information Vayentha gave him. Sims demands to see the video Zobrist sent to him after the virus was released.

Langdon and Sienna look for clues to the Palazzo Vecchio in order to find the painting "The Battle of Marciano", that contains similar writings to the one from The Map. As he recalls having been injected with something, he begins to experience more hallucinations. He also feels a burning sensation in his neck. Sienna assures him, if he were to contract the virus, then everyone around him would likely be dead. They meet up with Marta Alvarez (Ida Darvish), a friend of Langdon's. They examine security footage and find that Langdon and Ignazio robbed Dante's Death Mask. Langdon is once more under arrest as museum security and authorities chase him.

Langdon and Sienna race up to the attic to find the Apotheosis for Cosimo I. Vayentha tries to kill the two, but Sienna pushes Vayentha to the ground, causing her crash through the ceiling, and then to her death. Sinskey recognizes Vayentha when they arrive at the WHO.

Langdon and Sienna visit the Florence Baptistry to locate the mask. They follow the Paradise Twenty Five clue. Another hidden message is found on the back. Bouchard claims that he was the one who met Langdon in Cambridge before his amnesia. He also says that Sinskey is trying to spread the virus with her associates. Bouchard follows Langdon and Sienna, but Langdon soon realizes that Bouchard is lying and that Sinskey was the one he had met.

Simskey and Sinskey meet to discuss the virus' nature and to come together to prevent it from spreading.

Langdon discovers the virus is in the Hagia Sophia. Sienna then betrays Langdon and leaves him. She reveals that she was Zobrist’s lover. Langdon is then attacked by an attacker.

As he explained his philosophy and work, Sienna was able to see a flashback of them as lovers. Zobrist also left clues for Sienna to search in case something happened to him, as they used to treasure hunt together.
