Panel Discussion on Religious Exemptions From Vaccinations

What are the actual laws regarding mandatory vaccinations? What are the Jewish viewpoints and halacha on the topic? What is the process to obtain a religious exemption, and how does it depend upon whether the requirement is from a private business, school, or government agency?
