I started to sew the fold in the back when I do a one size fits all, After the sweatband is in and folded to the inside I put a line of stitches about 3/8" up across the back while I stretch that section out, it helps the cap look cleaner when you have the excess material from the bunching. I only do that on the one size fits all caps that have that style in the back. I'll put a picture of this up in my Etsy listing for the pattern.

When I make the large size it usually sits somewhere in the 7-3/8 to 7-5/8 US (A) range. Which is 22-3/4" to 24-1/2" head circumference.
When I make the Medium size it's usually fits around 6-7/8 to 7-1/8 US (A) sizes. Which is a 22 1/8" to 23" head circumference.
