Dear Father God, I know that I am a sinner, and I am so sorry, please forgive me.  I believe your son Jesus Christ of Nazareth died on the cross to save me from my sins and you rose him from the dead on the third day.  Lord, Father God I turn from my sins and invite you into my heart and my life as my Lord and Savior.  Thank you Father. I pray this in Jesus name.  Amen
