Fill the form out completely, make sure your request is straight forward and to the point. Make sure you put appropriate contact information, and for this particular request you will need to provide enough information for the department to identify you from other individuals with the same or similar names.

Recommended Wording (will update in future if more specific wording is necessary):

"I am seeking a record of all inquiries and request submissions made regarding the Vaccination records for myself, LIST YOUR NAME HERE, through the TN Department of Health Portal found at 'https://redcap.health.tn.gov/redcap/surveys/?s=LLJ83PT4PF'
This includes every date and time my name has been searched or a request submitted in my name, the number of searches and/or requests, name of Requester and, if available, any and all information logged about the requester in the database (ip address, etc). If you have any questions regarding this request please call me at YOUR PHONE NUMBER HERE."
