Methylene Blue and Serotonin Toxicity
Serotonin syndrome is a potentially life-threatening condition that can occur when high levels of serotonin accumulate in the brain, often due to drug interactions. Methylene blue, a redox dye, has been implicated in cases of serotonin syndrome when administered to patients taking serotonergic psychiatric medications, such as selective serotonin reuptake inhibitors (SSRIs), serotonin norepinephrine reuptake inhibitors (SNRIs), and clomipramine.

The FDA has issued warnings about the potential for serious central nervous system (CNS) reactions when methylene blue is given to patients taking serotonergic psychiatric medications.
These reactions can include mental changes such as confusion, hyperactivity, and memory problems; muscle twitching; excessive sweating; shivering or shaking; diarrhea; trouble with coordination; and fever.

Methylene blue is known to inhibit monoamine oxidase A (MAO A), an enzyme responsible for breaking down serotonin. This inhibition can lead to an accumulation of serotonin, potentially causing serotonin syndrome.

Cases of serotonin syndrome with methylene blue have been reported in patients undergoing parathyroid surgery, where methylene blue was used as a visualizing agent, and the doses ranged from 1 mg/kg to 8 mg/kg.
However, the risk of serotonin syndrome in patients taking serotonergic psychiatric medications who are given methylene blue by other routes, such as orally or by local tissue injection, or at intravenous doses lower than 1 mg/kg, is not fully understood.

Healthcare professionals and patients should be aware of the potential for serotonin syndrome when methylene blue is administered to patients taking serotonergic psychiatric medications. It is important to discontinue the use of methylene blue and other serotonergic medications if serotonin syndrome is suspected, as early recognition and discontinuation of offending agents are essential in managing the condition.

Serotonin syndrome should be suspected in patients with worsening mental status after methylene blue treatment, particularly if autonomic dysfunction is present.
