Ex-Black Hawk helicopter pilot believes three things went wrong in plane collision

Former Black Hawk helicopter pilot Elizabeth McCormick says she believes that three things may have led to the collision between an American Airlines flight and a US Army Black Hawk helicopter over the Potomac River.
