Cho Aniki: Kyuukyoku Muteki Ginga Saikyou Otoko, translated as "Super Big Brother - The Ultimate, Most Powerful Man in the Milky Way," debuted on the PlayStation, continuing the eccentric and over-the-top legacy of the series. Unlike its predecessors' hand-drawn anime-style visuals, this installment features digitized sprites, giving it a distinct look. A Sega Saturn version followed three months later. The game uses the engine from the original Cho Aniki and once again stars Idaten and Benten as the central characters.

The subtitle roughly translates to "The Ultimate, Invincible, Most Galactically Powerful Man," perfectly capturing the absurd charm the series is known for. The soundtrack boasts contributions from Koji Hayama, who returned to compose a few tracks, along with Don McCow, Taku Iwasaki, and Sanae Kasahara, offering a unique auditory experience that complements the game's outrageous aesthetic.
