MGS2 - Alert With Game Over If Discovered Turned On

Having a guard call in HQ after spotting another dead guard results in a caution under normal circumstances. In this room though, you get an Alert on top of that. The Game Over If Discovered condition only applies when transitioning into an Alert after being spotted, so this little glitch bypassed it.
