Lesson Learned:
1️⃣ Lack of Purging/Pipe Cleaning: The piping was not thoroughly purged and mechanically cleaned before starting the hydro testing of the tank.
2️⃣ People Present on the Tank During Critical Operation: No one should be allowed to work on the tank during hydro testing. Personnel must be removed from the vicinity of the equipment being tested, and the area should be RESTRICTED throughout the test duration.

Prevention Measures:
To avoid recurrence, always use your Permit-to-Work (PTW) System.
Key Features of a PTW System:
🔹 Clear identification of any work classified as hazardous.
🔹 Clear identification of personnel authorized for a specific job.
🔹 Clear identification of tasks, risk assessment, duration of permitted tasks, additional or simultaneous activities, and control measures.
🔹 Training and instructions regarding the issue, use, and closure of PTWs.
🔹 Monitoring and auditing to ensure the system works as planned.
When considering PTW for a job, the person issuing the permit must be aware of any other activity (either planned or underway) that may interact with the work covered by the permit.
