In 2009, a tragic accident occurred at a chocolate factory in Camden County, New Jersey, involving 29-year-old worker Vincent Smith II. While standing on a plank above an 8-foot-deep vat of hot melted chocolate, Smith lost his balance and fell into the container.

His co-workers immediately tried to shut off the machine and pull him to safety. Despite their quick response, they were unable to rescue Smith in time. The agitator, a device used to mix the chocolate, struck him, causing fatal injuries.

Smith remained trapped in the vat for about 10 minutes until emergency crews arrived. Tragically, by the time they managed to pull him out, it was too late. The chocolate in the vat, intended for Hershey bars, was heated to 120°F.
