Samira, a cancer patient nearing death, and her cat, Frodo, reside in a New York City hospice. One day, Sam is reluctant to go to Manhattan with the group until Reuben, a care worker, talks him into it. The gang sees meteor-like things slamming into the city while on the excursion. Attacks by hostile extraterrestrial beings on humans start soon after. Sam gets knocked out in the pandemonium that follows.

Later on, Sam and the other survivors awaken inside a puppet theater, together with Frodo. Henri, another survivor, motions for her to keep quiet. Everyone is silent. The bridges that lead out of the city are bombarded to stop the animals from leaving the island, while military helicopters overhead issue warnings to inhabitants to remain silent and hidden until rescues can be done. Henri accidently kills one of the survivors while trying to calm him down when he starts to lose control.

A devastated Sam takes Frodo and heads for Harlem after a beast kills Reuben. A military statement states that because the animals can't swim, boats are getting ready to remove residents from the southern part of the city. As groups of individuals begin to leave the buildings in the direction of the evacuation point, the sheer amount of people moving in unison alerts the animals, who then attack the group and start a stampede. Sam breaks away from Frodo and runs off in the other direction.

A law student from England named Eric meets Frodo and pursues him back to Sam. Sam tries to talk Eric into going to the evacuation site, but he's so shocked that he follows her back to her apartment, where she expects to get some painkillers. Eventually, Sam and Eric decide to journey together, barely escaping the beasts on their trip to Harlem. Eric stops at a neighboring drugstore along the route to pick up Sam's prescription. Sam shares with him her childhood memories of watching her late father perform jazz at a club and going to Patsy's for pizza with him afterward. Sam always wanted to do this before she passed away. In order to make Sam happy, Eric grants her wish by taking her to the club, getting pizza from a different store, and doing a magic card trick.

Later, Eric and Sam notice two boats nearby that are carrying people who are getting ready to flee. A group of critters gathers nearby as the two depart together. While Eric jumps into the ocean with Frodo in tow and is hauled up into a boat by Henri, Sam dashes out to divert the animals' attention by smashing car windows. In his jacket, he later discovers a note from Sam, thanking him for encouraging Frodo to live and advising Eric to keep caring for her.

Later, while listening to "Feeling Good" on her iPod, Sam strolls into an empty street. Sam smiles, accepting her impending demise, takes out her earbuds, blaring the music as a beast looms behind her.
