A big blackhead, perhaps a Dilated Pore of Winer!

This is a Dilated Pore of Winer, aka a big blackhead. These are usually a little more superficial under the skin, and can be pretty large. The keratin (skin cells) are oxidized, which give it a dark color. This man had this on his back for awhile, and came to see us because he knew that we were the pimple popping experts! ;) FYI, my left hand shakes when I use a needle to anesthetize the area on purpose. Shaking or pinching the skin when using a needle is a distraction technique. You feel less pain if you vibrate or pinch the area simultaneously.

Extracted using our DrPimplePopper comedone extractor, you can see that the sac was removed completely, and that the internal contents a are a little "sticky" and a little difficult to remove. I think we got it all!
