Earth has changed. Twenty years have passed since world leaders offered an unconditional surrender to alien forces. XCOM, the planet’s last line of defense, was left decimated and scattered. Optional Narrative Content
Make sure all boxes are unchecked so Rulers will spawn randomly instead of being locked to alien facilities and you will be able to build a frost bomb, boltcaster, etc at campaign start.

Additional MCM Configs and Instructions:

Squad Select: Uncheck "Upgrade Icons Only for Primaries." This will allow you to use gems on gauntlets and if you choose, attachments on secondaries.

World War Lost: You can freely enable/disable whatever you want in here.

LEB's Late Game Enemies: I've disabled the Riftkeeper. It makes missions extremely long and provides a very powerful item that enables extra squad members on Leviathan.

Community Promotion Screen: Uncheck "Display perks from unreached ranks" if you want the surprise of ranking of a new class you may not have played with.

Covert Infiltration: Check "Warn before expiration." "Hours Before Warning" is set to 3.

Quick Soldier Info - DISABLE taking headshots in tactical. I had crashes when QSI was attempting to take headshots upon starting gatecrasher. Seemed to happen mostly with a reaper in the squad. You are only able to change this setting once you're actually in tactical so if you're experiencing crashes upon mission load and you notice your screen flashes (that's QSI taking the photos), try a non-reaper start and deselect this option.
