The problem: the 8-32 x 1 hex cap screw loosens where it screws into the turn signal lever. The threads wear away slightly as the lever loosens and applies more stress on the screw threads instead of having a tight connection in the slot on the pivot. The raised section on the lever wears the switch plate pivot slot open so the lever never sits firmly which encourages the cap screw to back out loosening it all up. Eventually, the switch plate backs away from the base and the contacts for the brake & turn signal lights fail to meet.
The fix is to replace the cap screw with a new one, and the switch assembly . The new screw will fit the lever tighter and a new switch will have a slot that's not been worn open. Use thread locker and it works like new.
