I was asked if sugar alcohols can make you fat, by someone who’d eaten them as part of a keto meal and gained four pounds.

Sugar alcohols are compounds derived from sugar but they don’t trigger the same response in your body as sugar. They don’t spike insulin, and they have fewer calories than sugar. The three big side effects of sugar alcohols are bloating, gas, and diarrhea. These usually happen when you consume more than 25 grams of sugar alcohol. But some people are very sensitive, and can get diarrhea from less than half this amount.

Sugar gets absorbed in the small intestine, but sugar alcohols are dealt with by microbes in your large intestines. That’s where you may experience a reaction. The microbes ferment sugar alcohols, creating gas and a lot of water retention.

In other words, when you gain weight after eating sugar alcohols, it’s water weight not fat. You’re retaining water because of the reaction caused by microbes fermenting the sugar alcohols
