presents episode 946 | Dr Ben Bikman
Metabolic Health Podcast with Dr Dom D’Agnostino

Dr Ben Bikman
Insulin resistance has two facets...
1-expected reduced signaling due
to insulin not working as before
[this happens in some cells:
insulin resistance]
2-hyperinsulinemia [elevated insulin]
MUST HAVE BOTH TOGETHER

Our typical metabolic view is very
glucose-centric...
CGM [continuous glucose monitor]
-very useful clinical tool
-better than fasting glucose test to
get insights into metabolic health
Average patient, seeing physician
year in and out will
-gain weight
-have hypertension & getting meds
-some infertility

Physician ONLY CHECKS GLUCOSE...
-tend to be in normal range
-assumes no metabolic problem
"We MUST CHECK INSULIN"
-glucose is only symptom of
disordered insulin action
Dr D'Agnostino
-not part of standard blood test
-even med students confused why

Conventionally-trained clinicians
taught to only look at glucose...
If checked insulin,
-over years, insulin getting higher
-meaning: insulin resistance...
-but it keeps glucose in check
When insulin resistance gets into
-Alpha cells in pancreas
[they release glucagon]
-liver [disregulated glycogenesis]
-muscle [80% of glucose clearance!]
reduces ability by HALF
