How to Interpret an ABG?
It goes without saying, but knowing how to interpret an arterial blood gas is a very, very important skill for Respiratory Therapists. ABG interpretation is especially important in critically ill patients because it helps the healthcare team determine the best course of action in deciding how to treat the patient.

Here are the Basic Steps for ABG Interpretation:
-- Step 1 – Obtain and Run the ABG Sample
-- Step 2 – Determine if the pH is Alkalosis or Acidosis
-- Step 3 – Determine if the Issue is Respiratory or Metabolic
-- Step 4 – Determine if it’s Compensated or Uncompensated

What is an Arterial Blood Gas?
An Arterial Blood Gas, or ABG for short, is a test that measures the blood levels of oxygen (PaO2), carbon dioxide (PaCO2), and acid-base balance (pH) in the body. It’s a test that is used to assess how well oxygen is being distributed throughout the body and how well carbon dioxide is being removed.

Normally, healthy lungs move oxygen into the blood and push carbon dioxide out efficiently during inhalation and exhalation. This process is referred to as Gas Exchange. The body is receiving fuel in the form of oxygen while eliminating waste in the form of carbon dioxide. If a patient has breathing problems or a disease that affects the lungs, their ABG results would be abnormal.

What are the ABG Normal Values?
For you to better understand the key elements of an ABG test, it is important for you to know the definition of the values that are being measured.

-- pH – Refers to the acid-base balance of the blood.
-- Partial Pressure of Oxygen (PaO2) – Refers to the amount of oxygen that is in arterial blood.
-- Partial Pressure of Carbon Dioxide (PaCO2) – Refers to the amount of carbon dioxide that is in arterial blood.
-- Bicarbonate (HCO3-) – Refers to the total amount of CO2 that is transported in the blood.
-- Oxygen Saturation (SpO2) – Refers to the amount of hemoglobin in the blood that is saturated with oxygen.

ABG Normal Value Ranges:
-- pH: 7.35-7.45
-- Partial Pressure of Oxygen (PaO2): 75-100 mmHg
-- Partial Pressure of Carbon Dioxide (PaCO2): 35-45 mmHg
-- Bicarbonate (HCO3-): 22-26 mEq/L
-- Oxygen Saturation (SpO2): 94-100%
