IS THIS A FAILED Shower or Not?

In this video we tear out the second shower in this home. The first shower had no pan liner sealed to the three piece drain kit. The pan only had one coat of Red Guard and this caused the subfloor and joists to be rotted out. Tell me in the comments below, do you think this is a failed shower build and why?
