How to Write a SOAP Note

Writing a SOAP note can be seriously frustrating when you are first starting out. Do you seriously expect me to organize my thoughts into Subjective and Objective events when I am working with a patient? Most people find that mentally breaking apart their sessions into these categories is difficult. What is the Assessment? Why should I even care? Yeah, yeah, insurance, but isn't it basically the plan but watered down? HOW AM I SUPPOSED TO WRITE THIS!!! HELP!
