4.1 8.1 Schedule:
1. The scheme was behind schedule as the physical progress of work was observed to be lesser than the planned progress (Table 8, Figure 5).
• Planned Physical Progress: 90% as of Jan, 2023
• Actual Physical Progress: 65%
• Actual Financial Progress: 73%

At such a pace of work, the project cost and duration may be enhanced by 12% due to differences in the physical and financial progress and variance at completion as illustrated in Table 7 under Earned Value Analysis.
8.2 Cost:
2. The financial utilization of FY-2022-23 is observed to be 0% as shown in Table 5, due to non-utilization released funds under revenue portion of the PC-I which may result in delayed completion of the project.
8.3 Risks and Constraints:
3. The non-utilization of funds under revenue portion of PC-I may result in delayed completion and limited child protection facility in DG Khan.
8.4 Scope
4. The project informatory board required to be fixed at site of work.
5. A water gauge is required to be fixed, as shown in Figure 6.
6. Housekeeping is required while workers are required to provide the PPEs & Finishing works are also needed, as shown in Figure 7.
7. The operational building construction practices required to sustain the building against wearing, as shown in Figure 8.
8. The scrap material is required to be removed through coordination with administration and relevant department, as shown in Figure 9.
9. The expansion joint is required to be repaired while batten boards are needed to be used with steel plates to cover the joint, as shown in Figure 10.
10. The undulations in the plaster finishing are required to be fixed, as shown in Figure 11.
11. The compaction works are required near the ramps of the building, as shown in Figure 12.
12. The electromechanical equipment is required to be stored properly to avoid it wearing, as shown in Figure 13.
13. The efflorescence is required to be fixed while sweet water is needed to be ensured to avoid the efflorescence, as shown in Figure 14.
14. The compaction works are required before laying the tough tiles, as shown in Figure 15.
15. The sustainability and operational strategy of OHR of PHED is required to be formulated in collaboration with Home, PHE departments & local administration, as shown in Figure 16.

8.5 Health, Safety & Environment
16. The workers are required to be provided with Personal Protecting Equipment (PPEs).
17. The safety equipment such as safety ribbons, and safety signs are required to be fixed at the site. Housekeeping needed to be done while proper mobilization with porta cabins is required to be done at the site of work with the provision of safe drinking water to the human resources.
8.6 Quality Control
18. The quality control plans, quality checklists and methodologies are required to be documented at the site of work.
19. The engineering staff of contractor and consultant need to be present at site of work while a field laboratory is required to be established to control quality of work by contractor, consultant and client as well.
20. The observations as presented in the meeting held on 13th January, 2023 (12:00pm) under the chairmanship of Director General (M&E), P&D Board at the meeting room of Deputy Commissioner Office, DG Khan (Annex-A) are needed to be settled and a compliance report is required to be submitted to the office of DG (M&E), P&D Board, Lahore.
21. The data of major quality control tests such as field density test, load test, sieve analysis, compressive strength test, tough tile tests, steel yield strength test and cement quality test etc. could not be found at site. The field laboratory is required to be established and all relevant quality control tests are required to be performed as per standard specifications and proforma table given below.
