➡️ Driving Pressure
Driving pressure is a critical parameter in the management of mechanically ventilated patients. It represents the amount of energy being applied to the lungs during ventilation and is determined by subtracting PEEP from the plateau pressure.

🟦 Driving pressure = Pplat – PEEP

➡️ PEEP
PEEP is the pressure applied to the airways at the end of exhalation, while Pplat is the pressure in the lungs at the end of inspiration, just before the start of exhalation. The primary goal of mechanical ventilation is to maintain an adequate level of oxygenation and ventilation while minimizing lung injury. High driving pressure can cause injury to the lungs and increase the risk of complications such as barotrauma and volutrauma. Therefore, it is essential to monitor driving pressure and adjust the ventilator settings accordingly to prevent lung injury and improve patient outcomes.

The ideal driving pressure will vary depending on the patient's specific condition and the underlying lung pathology. The lower the driving pressure, the better the outcome for the patient. To minimize this risk, the driving pressure should be kept below 15 cmH2O. This can be achieved by using a lower tidal volume and adequate levels of PEEP, depending on the patient's static lung compliance.
