1949 paper on 16 patients with
severe, recurrent low back pain...
Doctor inserted needle into L4-5
disc, injected 2m saline
increased interdisc pressure
Results:
Reproduced their low back pain
in Every Subject
"Local anesthetic eliminated pain"
Proving the nerves were involved
[in outer part of annulus]

In every case in these experiments:
-pain was poorly localized
-many times described as
'muscular'
-due to efferent signals from these
nerves carried to brain
via sympathetic trunk
-it is visceral pain
[like angina or appendicitis]
-achy, poorly localized
-can be severe

pain signals carried along the
sinuvertebral nerves to the
sympathetic trunk
-then travel through L2-3
-carried by L2 nerve
-then to brain
"Pain from most of the lumbar
spine: carried along single nerve
This can be used for diagnosis
and treatment!

Example:
perform block of ganglion at L2-3
-stop signal from ascending
-stop pain experience
Study: 33 patients with low back pain
-had unilateral L2 nerve block on
the side of maximal pain
-during injection, 29 patients felt the
reproduction of their own pain
ALL PATIENTS reported REDUCTION in
back, buttocks, or groin pain
-26 had ALMOST ALL PAIN GONE...for 20 DAYS!
