no known pathoanatomical cause"
-evidence, from 1950s +
-most low back pain is from
disc pathology
-directly or indirectly
"Pain arising from the disc itself...
-outer 1-2mm [annulus fibrosus]
-have nerves that can cause pain
"Known for 70 years"

1949 paper by Swedish surgeon...
-in almost all cases he generated
low back pain in
'lumbosacral distribution'
"Irritating the outside of the disc
caused non-specific low back pain"

Paper documents a series of
cases, where a nylon thread was
passed through & around anatomical
structures in spine...
free loops protruding from skin!
-in one patient, it was looped
through outer part of L4-5 disc
-loop tugged on for 30 seconds:
"I have an ordinary backache"
-non-specific low back pain!

1991 paper: 193 patients
-operated on with local anesthetic
-before numb, used blunt
compression & electrical
stimulation to assess pain
response
Result
"Outer disc implicated in low
back pain"
-in 2/3 of patients, the stimulus
ALWAYS reproduced pre-operative pain

"Not just caused pain..,
but qualitatively reproduced
THEIR PAIN
-identical to their usual
low back pain"
Clear evidence that non-specific
low back pain is discogenic
[degenerative disc]
