Elderly driver NY:DWV5851 makes eye contact with me, then nearly hits me in bike lane

Driver looked right at me yet somehow doesn't see me, then makes an unsignaled lane change as I'm making a right turn to the bike lane, coming within inches of hitting my front tire and keeps on driving completely oblivious of how close he came to hitting me.
