Diabetic retinopathy is one of the most common microvascular complications in type 1 and type 2 diabetes and is the leading cause of severe vision loss that eventually leads to blindness. Increasing evidence suggest that inflammation induced by uncontrolled, chronic hyperglycemia plays a pivotal roal in retinal perturbations associated with diabetes. According to studies, chronic hyperglycemia activates alternative pathways of glucose metabolism, such as oxidative stress and non-enzymatic protein glycation, which leads to the formation of advanced glycation end-products or AGEs. AGEs consist of glycated proteins or lipids combined with sugar during exposure in the blood stream. These compounds are formed throughout the course of life, but in diabetes there is an increased rate of AGEs formation, which causes the activaiton of pro-inflammatory molecules and vascular endothelial dysfunction, leading microvascular occlusion. Microvascular occlusion then leads to retinal ischemia, in which irreversible cell death is caused by lack of oxygen supply in the retina. Accumulation of oxidative stress is also considered a main pathogenesis of diabetic retinopathy. It leads to vascular dysfunction, mitochondrial damage, inflammation and lipid peroxidation that cause cell and tissue damage and death. In order to treat diabetic retinopathy, strict glycemic control is the best way to help control diabetic retinopathy. A combination of anti-inflammatory compounds and antioxidants including NAC, alpha-lipoic acid and lutein, is also an eccelent adjuvant therapy to slow or stop the progression of this disease into blindness. NAC has been shown to protect the retina of diabetic mice against reactive oxygen species and inflammation that are damaging the retina. While alpha lipoic acid has been shown to attenuate retinal injury in diabetic mice through its potent antioxidant effects.
