P-COS is a common endocrine disorder that has major metabolic and reproductive morbidities. P-COS is associated with insulin resistance, which is worsened by obesity and hyperandrogenemia Insulin resistance is also the major cause of infertility in 74% of P-COS women. Infertility is the consequence of chronic anovulation accompanied by menstrual abnormalities. Insulin resistance also seems to be the main predictor of metabolic syndrome in P-COS, characterized by dyslipidemia, central obesity, hypertension, type 2 diabetes mellitus, and coronary artery disease. Due to the main role of insulin resistance in the overall health of P-COS women, doctors would suggest patients lose weight because obesity worsens insulin resistance. They also prescribe insulin-sensitizing drugs, especially metformin, which may improve insulin resistance. Improvement of insulin resistance restores the hormonal status and the ovulation process. There are also other primary treatment options for improving insulin resistance and associated morbidities of insulin resistance, aside from metformin, such as inositol. Inositol is a naturally occurring sugar abundant in animals and mammals. Myo-inositol is the most common type of inositol found in nature. It is more considered an insulin-sensitizing supplement more than a drug, that may decrease insulin resistance. It improves the overall condition of P-COS patients. It decreases hyperandrogenism, improves the menstrual cycle, and increases good-quality oocytes. In a prospective study, women who received myoinositol and folic acid experienced 82% of the cases of anovulation. In another study, administration of Myo-inositol led to a higher number of pregnancies in women with P-COS in comparison with the administration of metformin in another group of patients.
