Sildenafil citrate is a medication that is used in the management and treatment of erectile dysfunction and pulmonary arterial hypertension. In a recent study, sildenafil has been found to be associated with reduced risk of Alzheimer's disease. Sildenafil citrate inhibits the actions of phosphodiesterase 5, which degrades cGMP. Upstream of cGMP allows the conversion of L arginine by the enzyme nitric oxide synthase or NOS into nitric oxide. In Alzheimer's disease, the activity of NOS, nitric oxide and cGMP pathway is impaired. This NOS, nitric oxide and cGMP pathway could be an effective therapeutic target in Alzheimer's disease for multiple reasons. CGM P increases the expression and activity of peroxisome proliferator-activated receptor gamma coactivator 1 alpha or PGC-1 alpha proteins, which are also significantly lower in Alzheimer's disease patients hippocampi. PGC-1 alpha activity suppresses the A beta generation and upregulates multiple antioxidant genes, including mitochondrial manganese superoxide dismutase when deacetylated sirtuin 1. Alzheimer's disease, alongside erectile dysfunction and pulmonary hypertension, is characterized with lower levels of cGMP. Blocking the activity of phosphodiesterase-5-inhibitor increases cGMP levels.
