PICCI insertion with the Sherlock TCG tip locator. Intracavitary ECG tip confirmation is such a game changer for CVC placements. Intracavitary ECG, EKG is more accurate and precise In locating the catheter tip than x-ray confirmation… When it works. The magnetic part of the technology is left to be desired but the ECG still works great. CVC intracavitary ECG tip confirmation is ineffective only when there is a significant and prominent atrial fibrillation. The CVC Intracavitary ECG tip confirmation depends on the Electrical conduction of the P-wave being fired by the SA node. Metaphorically the catheter becomes a microphone while the heart is the singer/instrument. The closer the catheter tip gets to the SA node the “louder” amplification of the P wave increases. The SA node is anatomically at the cavoatrial junction. The CAJ is the most optimal position for most CVC’s.
